Dear Parents and Guardians: This week (September 14), all Seaforth students will take home the following forms which require your attention. Important – Please Complete, Sign and Return the forms (to the classroom teacher) by September 20, 2023.
-Student Verification Form: shows information on student profile. Please review and update the form paying particular attention to telephone numbers and email addresses. Simply cross out incorrect or outdated information and clearly print new or updated information. Be sure to sign and date the form.
-Medical Information form. Thank you for completing and signing this form each year. Note: Students with previously reported specific medical needs (eg. Anaphylaxis Emergency Action Plan, Asthma Action Plan, etc) have been provided with a complete copy of the current information on file. Please review the documentation. If there are no changes, sign and date the bottom of each page and return.
-Student Emergency Release Form. Thank you for providing this important information.

COMING SOON: Burnaby School District has an online process for consent and permission forms. Instructions will be provided soon but in anticipation of this process, please note each family will require the following information:
- your child’s PEN (Personal Education Number) please see any copy of your child’s report card;
- your child’s birth date; and
- current parent email address attached to the child’s profile (please check/update the Verification Form).
